The Buurtzorg nurse team develops individualized care plans for the discharged elders, provides care and nursing services, reduces the challenges and difficulties faced by the elders after discharge, and allows them to continue living in the community. Reduce the risk of repeated visits to and from the hospital.
Through holistic health assessment, we can understand the physical condition and cognitive ability of the elderly, and provide basic care and daily life support services for the elderly who need assistance
Create a personalized care plan for the elderly to help patients enjoy professional and comprehensive nursing services at home

Residents of Wong Tai Sin aged 60 or above (required by partner organisations)
Services
Nursing plans include but are not limited to the following areas:
chronic disease management
Post-Stroke Rehabilitation Care
Medication guidance and self-improvement
Linking community care networks to support long-term care needs
Fall risk assessment and prevention, home safety improvement guidance
Referral to community resources in a timely manner to ensure that participants receive appropriate living support
Disease Prevention and Health Guidance
Provide a hotline for participants and their families to contact the nursing team to follow up on their rehabilitation progress and needs

The frequency, content, and health management plan of home visits must be evaluated by professional social workers and nursing staff and determined according to the needs of the participants
* Program duration 6-12 weeks
service charge
The fee for nursing care service per home visit is HK$80
* Exemptions are available for those currently receiving Comprehensive Social Security Assistance (CSSA) scheme or seniors with higher OALA
Service application method
This program is not open for recruitment and needs to be referred by partner institutions
