Professional Outreach Team
The project's outreach team, consists of registered nurses (general and psychiatric), enrolled nurses, social workers, dietitians, and health workers, has provided services to more than 1,600 elderly. We work together towards a common goal of meeting the diverse needs of the elderly using a community-care approach. For every client, team members of the interprofessional team will communicate and collaborate effectively with each other to create an individualized health care plan. Case managers will coordiante the implementation of the care plan and make sure that the elderly clients understand their plan of care.
