Community-based health management program
Service Objectives
Our team consists of social workers and nurses. We conduct comprehensive geriatric health assessments in the community, develop community-based health management programs to promote the development of community health services, and provide daily living assistance for the elderly. This program enables the elderly to remain living in a familiar community, so as to help them "age in place".

Using comprehensive health assessments to determine the health status and cognitive ability of elderly participants, and help them establish their health visions

Develop a community-based health management plan for the elderly, and jointly set health goals and improvement plans to help them achieve their health vision.

Strengthen the self-efficacy of elderly persons and continue to maintain or improve their health without having to enter institutions prematurely.

Service users in Kwun Tong District referred by Hong Kong Christian Service Bliss District Elderly Community Centre
Scope of Service
The comprehensive geriatric assessment includes, but not limited to the following:

Physical Examination

Assessing chronic disease management and medication management

Leveraging the participants’ formal and informal care network to support individual’s long-term care needs

Fall risk assessment and prevention, as well as guidance on improving safety at home

Timely referral to community resources to ensure elderly participants can receive appropriate daily living support services.

Number of Sessions
The frequency of home visit, content, and health care plan may vary from case to case, depending on the initial assessment and individual needs.
*Program will last for 6-12 weeks
