The team has been committed to providing the best care possible for the elderly living in the community.

Health Management and Health Coaching

Many elderly individuals suffer from various chronic diseases, but they may not have the knowledge, skills, and confidence to effectively manage their conditions. Over the past two years, our project team has created personalized health care plans for elderly participants. Based on their self-care ability, evidence-based interventions such as health coaching and nurse-led program were provided to help elderly individuals manage their chronic diseases effectively and improve their health. The following are examples and scope of our services:

Medication Guidance

The team will educate elderly individuals about medication therapy, including, but not limited to medication effects, recommended dosage, proper distribution and storage method, and timing of medication administration. Many elderly have cognitive impairment and age-related memory loss. To help elderly clients take the right medication dose of the right pills and injections at the right time, our team created a reminder and placed it in a prominent place so that it can be easily viewed.

Self-health monitoring

Checking the blood glucose for the elderly and teaching them how to use the glucometer to self-monitor their blood glucose levels.

Ageing decreases the functional capacity of the elderly and increases the risk of decline in mobility. To slow down progression of mobility decline that comes with ageing, our team will regularly provide walking training for the elderly. In addition, many elderly persons are unable to go outdoor to exercise or perform moderate-to-vigorous intensity physical activity due to various health problems. To raise the elderly's interest level in doing physical activity, our team will teach the elderly how to perform some simple stretching and home exercises.

Visiting the elderly in their homes

Some elderly individuals may not be able to go to nearby District Elderly Community Center due to various reasons. Our outreach team will conduct home visits to assess the health needs of the elderly, and provide appropriate advice to improve their health. After completing the assessment, the nursing team will immediately provide a brief report to the elderly, so that they can understanding their current health situation and be able to report their condition to their physicians during regular medical checkups.

Housing Conditions of Elderly Persons

The housing conditions of elderly persons

Elderly people living in poor housing conditions will be referred to partnering organizations. Community-based social services, such as household maintenance, household cleaning, and pest control, will be provided by social welfare organizations. Elderly people will also be educated on how to use walking aids correctly to prevent fall and preserve mobility. Besides, elderly persons will also be recommended to reduce clutter in their home in order to increase their activity space.

Collaboration with Partnering Organizations

Collaboration with partnering organizations on a regular basis

Many elderly persons lack awareness of their health conditions. In order to increase their elderly's their health awareness and prevent illnesses, our team will regularly collaborate with different organizations to host health seminars and conduct health assessment training courses for volunteers. In addition, given that most of the elder people do not undergo regular physical examinations, our team will regularly collaborate with partnering organizations to measure the vital signs of the elder people, so that they can have a better understanding of their health conditions, and be able to make improvements and seek medical assistance if necessary.

Outreach during COVID-19

Outreach during the COVID-19 pandemic

In the beginning of the COVID-19 pandemic, there was a shortage of medical supplies in Hong Kong. From February to May of 2020, our team identified elderly persons with poor social support in the community, and provided them with anti-epidemic items through home visits. We educated the elderly about the COVID-19, with information such as the mode of transmission, the proper way of using alcohol-based hand gel and precautions, and the proper technique of wearing a mask.

From February to May 2020, the team visited the elderly on four different occasions to distribute anti-epidemic items. For the first visit, all of the supplies were donated by our team members. For the remaining visits, the supplies were donated by kind-hearted individuals and organisations in the community. On behalf of the elderly, we would like to express our sincere gratitude to every donor for their donations.
