Service Philosophy

This projects aims to improve the health of the elderly and strengthen their self-management skills to achieve the long-term goal of "ageing in place" through the following elements.

Personalized

We believe that every elderly person has different needs and our team will conduct a comprehensive assessment for each participating elderly. Based on the initial assessment, a personalized care plan will be provided for each elderly to address their unique health needs.

Community oriented

Our team will collect information in the community and assist the elderly participants in connecting with different community resources, such as neighbourhood support, social services and social enterprises, in accordance with their needs. We hope the elderly can make good use of the existing resources in the community to help them improve their health and wellbeing.

Professional Team

We are a team of professional nurses and social workers, and we understand that the challenges faced by elderly persons are multifaceted, so our team will not only provide nursing care, but also services to improve their self-management skills, mental health, social life, and the home environment.

Medical-social collaboration

The service team is fortunate to have not only support but also active participation from public hospitals, social welfare organizations, and social innovation organizations. Through collaboration between teams, and by combining medical and community resources, we are addressing the overall health, emotional and daily needs of the elderly, with the aim of improving their health and quality of life.
