Service Objectives
To prevent premature institutionalization in residential care homes and help the elderly maintain independence in a familiar community, our Buurtzorg nursing team will develop personalized home care plans and provide nursing care services for community-dwelling elderly, which enables them to continue "age in place" in a familiar community.

Understand the health status, cognitive ability of the elderly through comprehensive assessments, and provide basic care and daily life support services for the elderly in need.

Basic nursing care (wound dressing, catheterization)

Chronic disease management

Diseases prevention and health coaching

Stroke rehabilitation care

Medication adherence and self-management

Fall risk prevention (including suggestions for improving your home environment)

Assist in connecting with different community resources (including neighbourhood support, social welfare organizations, social enterprises, and others) to support elderly persons' long-term care needs

Make referrals to community services to help the elderly live independently

Service Charge
HKD$80 per home visit
Elderly persons aged 65 or above are eligible to use the Health Care Vouchers
Number of Sessions
The frequency of home visit, content, and health care plan may vary from case to case, depending on the initial assessment and individual needs.
* Program will last for 6-12 weeks
